transmission questions

While driving my car my transmission started making a bad noise it made a noise in every gear but 4th while driving but doesn't make the noise in neutral. There is no grinding and it shifts smoothly I checked the fluid and I only had half a quart I added fluid it took almost 3 quarts and its still making the noise.
Any ideas what thia could be?

Well it tuns out that my input shaft bearing was bad....
But the transmission has been replaced and the rollas back on the road again.
